Health Services Beginning of the Year Parent Letter (Spanish)

1.  SchoolCare (formerly known as Caredox)

This is the platform used by the school nurse for electronic health records.  Parents/guardians can access these records at https://secure.caredox.com/c/accounts/login.  Email for login must match the email we have on file in Powerschool.  Parents/guardians can begin updating their students' health information on 8/16/23.  A reminder email will also be sent by SchoolCare within the first two weeks of school.

2.  COVID 19 Guidelines

We will go into the 2023-24 school year operating the same as we ended last school year.  Going forward, school administrators, staff, and families should refer to the CDC’s Operational Guidance for K12 Schools for information on how to lower the risk of COVID-19.

For NEW Students...

1.  After registering, an email should arrive that will allow you to fill out the health information for your student. If your student has current health concerns or is taking medication, please contact the school nurse as soon as possible by calling the school (828-264-8481).

2.  Immunization Requirements

Parents/guardians requesting an Immunization Exemption need to be referred to https://immunization.dph.ncdhhs.gov/schools/ncexemptions.htm for requirements.   

3.  NC Health Assessments  This is required for all NEW students that have never attended a NC Public School or NC Charter School as well as Pre-Kindergarten and Kindergarten. There is not an exemption for Health Assessments.  

The NC Health Assessment Form for K-12 must be completed by the parents/guardians and physician.  A physical is required, so physician signature is a must.  Forms can be found at: 

For PRE-K Students...

1.  Pre-K students can use the NC Health Assessment Forms or the Children's Medical Report Form which can be found at https://ncchildcare.ncdhhs.gov/Portals/0/documents/pdf/C/Childrens_medical_report.pdf  

2.  PRE-K students MUST have a Dental Screening Form, no matter which Health Assessment Form is used.  This form can be found at:
