For NEW Students...
1. After registering, an email should arrive that will allow you to fill out the health information for your student. If your student has current health concerns or is taking medication, please contact the school nurse as soon as possible by calling the school (828-264-8481).
2. Immunization Requirements

Parents/guardians requesting an Immunization Exemption need to be referred to https://immunization.dph.
3. NC Health Assessments This is required for all NEW students that have never attended a NC Public School or NC Charter School as well as Pre-Kindergarten and Kindergarten. There is not an exemption for Health Assessments.
The NC Health Assessment Form for K-12 must be completed by the parents/guardians and physician. A physical is required, so physician signature is a must. Forms can be found at:
For PRE-K Students...
1. Pre-K students can use the NC Health Assessment Forms or the Children's Medical Report Form which can be found at https://ncchildcare.
2. PRE-K students MUST have a Dental Screening Form, no matter which Health Assessment Form is used. This form can be found at:
